Request a COVID Recovered Banner

Ensure your patient meets the discontinuation criteria

Place a consult order:

  • Inpatients: “Inpatient Consult to HEIP/IC for COVID Recovered Evaluation.” 
  • Outpatients: “Ambulatory Referral to HEIP for COVID Flag Removal.” 

Report a COVID Positive Patient

Submit the required information. Requests to HEIP via phone, email, Voalte, etc. will not be accepted and may result in delays to updating patient medical records. 

Report a COVID Positive Employee

Have the employee take the daily UCSF health screener and “fail” it to report the positive result. This will trigger a review by OHS and HEIP.

Request a COVID Exposed Banner for a Patient

Submit the required information. Requests via phone, email, Voalte, etc. will not be accepted and may result in delays to updating patient medical records.

Visitor/Caretaker Requests

Please review the current visitor guidance; request review by the Visitor Exemption Committee via http://tiny.ucsf.edu/VisitorRequest.
